HOMEOWNERS & DWELLING PROPERTY APPLICATION

 

The electronic application is the most effective method to apply to the Illinois FAIR Plan, however if manual completion is required, click here to print the application, complete manually and mail with supporting documentation.

 

ELECTRONIC HOMEOWNERS & DWELLING PROPERTY

APPLICATION

 

The electronic application offers various options for completion.

  • To assure the fastest turnaround, the FAIR Plan Homeowners and Owner Occupied Dwelling Property application can be completed online and submitted electronically with the necessary supporting documentation and the applicant's down payment.

    Payments will be made through an electronic funds transfer from an applicant's checking or savings account. Credit card and money order payments are not available at this time. 
  • As an alternative, when an electronic payment will not be included, such as a non-owner occupied dwelling property application being submitted for quotation or a mortgagee billed policy, the application can be completed online and submitted electronically with supporting documentation.  
  • As an additional alternative, the application can be completed online, printed and submitted by mail with supporting documentation . Right click anywhere on the application after completion and select "Printable Version".

ILLINOIS FAIR PLAN REQUIREMENTS FOR (1-4 FAMILY)

HOMEOWNERS & DWELLING PROPERTY COVERAGE

 

All applications are subject to prior underwriting approval. PRODUCERS DO NOT HAVE BINDING AUTHORITY.

 

HOMEOWNERS & OWNER OCCUPIED DWELLING PROPERTY PROGRAMS

 

If ALL of the following is received and all eligibility requirements are met, coverage can go into effect at 12:01 AM the day following FAIR Plan receipt or a future date. Incomplete applications will be returned.

  • Fully completed and signed Application. If the application is submitted electronically, the last 4 digits of the applicant’s and the producer’s Social Security numbers will be accepted as their electronic signatures and will have the same legal effect as if they had signed their names.
  • When the applicant resides in a mandatory Mine Subsidence Insurance county and is waiving Mine Subsidence Coverage, a manually signed Mine Subsidence Waiver (FP 344) must be included with the application.
  • Current photo of front and rear of building (faxed copies not accepted) (not required for HO 4 or HO 6).
  • Minimum of 20% of annual premium.
  • Replacement cost calculation (HO 2 or HO 3 only).
    - HO 2 - Coverage amount minimum of 80% of replacement cost
    - HO 3 - Coverage amount 100% of replacement cost

A survey will be completed after policy issuance to confirm underwriting requirements are met.

 

NON-OWNER OCCUPIED DWELLING PROPERTY

 

Coverage is not available until a survey of the property has been completed, coverage has been approved and a minimum 20% premium deposit has been received by the FAIR Plan.

 

Upon receipt of completed and signed applications, the FAIR Plan will survey the property. If the risk is found acceptable, a premium quotation will be issued.

 

Coverage can go into effect at 12:01 AM the day following FAIR Plan receipt of the premium, or a future date.

 

Liability and theft coverage's are not available under the Dwelling Property program.

 

A policy may be requested if the FAIR Plan has not issued a premium quotation or a denial of coverage within 21 days of receipt of the application.
